Under administrative direction of Value Based Specialist (VBS) in collaboration with Bloomington practices helps patients understand treatment plans and navigate resources . Duties/Responsibilities: Help High need patients understand treatment plans and following provider instructions. Care coordination with patients to navigate medical care and non-medical need resources for example food, transportation, housing.
Assists patients with navigation of transition of care from inpatient to ambulatory and/or follow up appointments from emergency department visits. Assists VBS in closing care gaps, scheduling and tracking of annual wellness visits Assists Practices in chronic care management Communication between providers/patients/VBS/ACO team
/Abilities: Excellent verbal and written communication skills. Excellent interpersonal and customer service skills. Excellent organizational skills and attention to detail.
Excellent time management skills with a proven ability to meet deadlines. Proficient with Microsoft Office Suite or related software. Especially Excel Education and
