About the role

The Admissions Liaison is responsible for collecting and inputting information gathered via detailed patient chart review, communication with patient, communication with patient family and communication with medical team in order to present the data to the Medical admissions team, in order to determine the appropriateness of admission to the acute rehab unit and whether the patient meets acute rehab unit admissions criteria.
