Admissions Coordinator - Home Health

JOB SUMMARY To facilitate the referral/intake process for potential patients/clients; including receiving referrals; ensuring benefits are verified; gathering necessary medical information; orders; and face to face documentation ; and coordinating care with the patient/client; their family; attending physician; and the referral source for a smooth transition into agency care. JOB REQUIREMENTS Minimum Education Associate degree in Nursing Minimum Work Experience 1 year of experience as a Licensed Vocational/Practical Nurse or Registered Nurse; home health experience preferred Required Licenses/Certifications Registered Nurse or Licensed Vocational/Practical Nurse CPR Current Texas Driver’s License
