Position Statement: As a member of the Community Health Team embedded within North Country Primary Care, the Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW) provides integrated outpatient behavioral health services to adult patients. The LMHC or LCSW works collaboratively with primary care providers, nurses, care managers, and interdisciplinary healthcare team members to support whole-person care, improve access to behavioral health services, and address the mental health and psychosocial needs of adults across the lifespan.
Key
Provide individual counseling and brief therapeutic interventions to adult patients experiencing a variety of mental health concerns, including anxiety, depression, stress-related conditions, trauma, grief, substance use concerns, and behavioral health challenges. Conduct comprehensive intake assessments, develop individualized treatment plans, and monitor client progress using evidence-based therapeutic modalities, including Cognitive Behavioral Therapy (CBT), mindfulness-based interventions, solution-focused therapy, motivational interviewing (MI), and trauma-informed approaches.
Collaborate closely with primary care providers and medical staff to support an integrated model of care and ensure coordinated treatment planning. Participate in behavioral health screenings, case consultations, and multidisciplinary care team meetings. Assist patients in navigating healthcare and community resources, including referrals to specialty mental health services, social support, and other appropriate resources.
Provide clinical and/or psychosocial assessment and intervention consistent with the clinician’s professional training, licensure, and scope of practice. Maintain accurate and timely clinical documentation in compliance with ethical, legal, and practice standards. Advocate for patient needs and provide referrals to appropriate community resources as needed.
Stay current with best practices in adult mental health treatment through continuing education, professional development, and ongoing clinical learning. Knowledge, Skills and Abilities: Master’s degree in counseling, Psychology, Social Work, or related behavioral health field from an accredited institution.
Current, active licensure as a Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW) in the state of Vermont. Minimum of 2 years of experience providing mental health services to adult populations preferred. Experience working in integrated primary care, outpatient behavioral health, or healthcare setting preferred.
Strong clinical skills in evidence-based therapeutic practices, assessment, and treatment planning. Knowledge and experience addressing common adult behavioral health concerns, including mood disorders, anxiety disorders, trauma, adjustment disorders, and chronic illness-related emotional needs. Excellent interpersonal, communication, and organizational skills.
Ability to work collaboratively within a multidisciplinary healthcare team. Commitment to patient-centered, culturally responsive, and trauma-informed care For LCSW candidates, knowledge and experience with psychosocial assessment, social determinants of health, care coordination, and community resource navigation is preferred. For LMHC candidates, knowledge and experience with clinical counseling, behavioral health assessment, psychotherapy, and evidence-based therapeutic interventions is preferred
