The clinical pharmacist is responsible for the safe, effective, evidence-based, and cost-effective selection and utilization of medications for the patients in which they serve. The clinical pharmacist uses advanced knowledge, skills, and professional judgment in all aspects of his or her work. The clinical pharmacist serves as a patient advocate when planning and implementing pharmaceutical care within a multidisciplinary rounding team, collaborating with other healthcare professionals in a knowledgeable and professional manner.
In addition, the advanced clinical pharmacist is expected to lead pharmacy services within the hospital by serving or chairing hospital committees, presenting at conferences and contributing to publications, and providing regular, formal education to pharmacy and interdisciplinary teams. The advanced clinical pharmacist is responsible for one or more specialty services within the hospital, such as infectious disease (ID), critical care, and pediatrics, among others.
