About the role

The Nurse Practitioner will provide comprehensive outpatient follow-up care for patients discharged from the hospital after being diagnosed with and treated for a stroke. The role will include assessing recovery, coordinating ongoing care, and addressing the patient’s neurological and overall health needs. A background in neurology is preferred, while experience in gerontology is a valuable plus.
