Job Summary The Advanced Practice Nurse, Hospital at Home (APN-HaH) is accountable for providing comprehensive patient care congruent with role and approved population (Family/Individuals across lifespan and Adult/Gerontology) through systemic assessment, diagnosis, treatment, management, and follow up affecting direct patient care under the supervision of HaH Clinical Medical Director.
The APN-HaH serves as a clinical expert and consultant to support the healthcare system in providing patient-centered, culturally-competent, outcome-focused therapeutic care within the management of specified populations through independent and collaborative clinical management. This APN-HaH provides high-quality patient care to various patient populations within the framework of the Hospital at Home Program model. The APN-HaH will be accountable for planning, implementing, evaluating and communicating all phases of patient's plan of care to the Medical and nursing teams. This APN is responsible for facilitating an environment that supports professional practice and empowers all interdisciplinary healthcare workers to provide safe, effective, compassionate, and efficient patient care.
Degrees: a) Bachelor's Degree b) Master's Degree: Graduation from an accredited School of Nursing with a Master's degree in Nursing with completion of a university-based program for Advanced Practice Nurse required.
