** $10,000 Sign on Bonus Opportunity ** Great opportunity for a Nurse Practitioner or Physician Assistant to join our Team at West Hay Family Medicine, located in Decatur, IL! This is a Family Medicine Advanced Practice Provider position, responsible for treating patients of all ages across the lifespan (pediatric through geriatric). This role will facilitate the delivery and evaluation of evidence-based comprehensive care to patients.

Licensure/Certification/Registry: - One of the following is required: - Must possess and maintain licensing as an APRN as required by Illinois State Law; - Must possess and maintain licensing as a PA as required by Illinois State law; - Appropriate certification as a APRN or PA, as determined by department. - Maintain IL Controlled Substance license (Must be obtained within 3 months of hire if not held at time of hire). - Maintain DEA license (Must be obtained within 3 months of hire if not held at time of hire) in positions where required. - Valid CPR certification required. - ACLS where required.
- Minimum of 1-year post-graduate as a mid-level provider preferred Pay Transparency $0.01 - $0.01 Additional compensation in the form(s) of participation in our annual incentive program are dependent on role.
