The Nurse Practitioner is accountable to facilitate a quality hospital experience and outcome for patients within a cost-effective time frame. The Nurse Practitioner I will be responsible to the Medical Director of Patient Services and responsible for overseeing the progress of a specific caseload of patients across the care continuum to include pre-admission and post-discharge when feasible. Acts as an educator, consultant, and liaison to healthcare providers caring for the patients across the care continuum.
Demonstrates a fundamental grounding in nursing theory and practice with mastery of knowledge within a defined content area. Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings. Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model.
Oversees care coordination in acute, home care, and ambulatory care settings as feasible. Assists the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support, education and encouragement Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics. Provides direct care to patients in acute care, ambulatory care and home care settings.
Performs advanced practice skills under the supervision of the Medical Director. Provides expert consultation for clinical issues promoting increased knowledge and skills for the consultee. Licensure as Registered Nurse in the State of Rhode Island by the Rhode Island Board of Nursing or licensure as a Registered Nurse in accordance with the Nurse Licensure Compact agreement of the National Council of State Boards of Nursing. BASIC KNOWLEDGE: Masters of Science Degree, Graduate of accredited Nurse Practitioner program. Specialty certification required.
Baccalaureate in Nursing Certified in Basic Cardiac Life Support (BLS). Demonstrates knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.
Two years related experience as RN in acute care hospital setting and preferred 1-2 year's experience as a nurse practitioner. WORK ENVIRONMENT AND PHYSICAL
