✓Requirements
Education
✓Doctor of Pharmacy (Pharm.D.) degree from an accredited college or university.
✓Successful completion of an accredited PGY1 Pharmacy Residency program
Qualifications
✓Demonstrated clinical experience in either a hospital-based or ambulatory care pharmacy practice setting.
✓Strong knowledge of pharmacotherapy, chronic disease state management, and evidence-based clinical practice guidelines.
✓Ability to assess, monitor, and optimize medication therapy to improve patient outcomes and medication safety.
✓Excellent interpersonal, communication, and patient counseling skills, with the ability to effectively communicate complex clinical information to patients, caregivers, physicians, and other healthcare professionals.
✓Proficiency with electronic medical records (EMR) and clinical documentation systems.
✓Strong critical thinking, problem-solving, and organizational skills with the ability to work effectively within a multidisciplinary healthcare team.
