You’ll need
Licensure
Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP) preferred
Qualifications
Previous experience with the clinical management of primary care based chronic diseases preferred
Completion of a PGY2 Ambulatory Care Residency, completion of a PGY1 Pharmacy Residency with strong ambulatory care focus, or equivalent ambulatory care experience preferred

About the role
Luke’s Ambulatory Pharmacy Department includes pharmacists embedded in clinics across the health system within the following specialties: internal medicine, family medicine, endocrinology, anticoagulation, heart failure, lipidology, cystic fibrosis, and pediatric specialties.
