Manages comprehensive and individualized care through the inpatient or outpatient phase in collaboration with the pulmonologist by utilizing the best available evidence to guide practice interventions. Enhance and implement the delivery of optimal patient care and evaluate compliance to improve patient outcomes. Provide consultation to the pulmonary physicians and healthcare team; patients and families.

Utilizes teaching, learning theory and appropriate teaching strategies in the provision of education to patients and familiesCollaborate with the Clinical Manager and Executive Director, Service line to serve as leaders, change-agents and advocates at all levels to create innovative solutions for pulmonary patients as well as assisting with quality measuresParticipate in Multi-Disciplinary RoundsCommunicates effectively with patients, families, physicians and the multidisciplinary healthcare team to assure the delivery of efficient and coordinated careResponsible for reconciling patient discharge medicationsMandatory phone calls on all patients that are discharged to monitor care on a consistent continuum.

Physical RequirementsThis job can be physically demanding at times requiring stooping, sitting, standing, and walking for long periods of time. The team member will need to frequently assist lifting patients and heavy equipment.

Must be able to work in a stressful environment, work independently, making sound decisions, detail oriented, alert and self-motivated. Ability to push/pull up to 40lbs, carry and lift up to 50lbs.
