Position Details Facility: BayCare Health System, Administration Lakeland - HomeCare Location: HomeCare Lakeland- 20 Lakewire Drive, Lakeland, FL, 33815 Status: Full Time, Exempt: Yes Shift Hours: 8:00 AM - 5:00 PM Weekend Work: Occasional On Call: Yes

Clinical oversight and direct supervisor over patient care supervisors providing guidance, training, and support to ensure consistent delivery of high-quality care across the organization. Leads the agency's quality assessment and performance improvement program in alignment with the Quality Manager for BayCare Home Care. Coordinate responses to internal audits and external regulatory surveys, including developing Plans of Correction and ensuring evidence of submission.

Participates in staff development, mentoring patient care supervisors, assist with onboarding, competency validation and performance evaluations.

Certifications/Licenses: State of Florida Board of Nursing License Required

Associates degree Required

2 years of supervisory experience in post-acute/Home Care line of services. OASIS Accuracy QAPI preparation of Corrective Action Planning Home Health Regulations (CMS) Survey preparedness Management of team member, team member experience, patient experience, and associated KPI metrics Any equivalent combination of education and experience that provides the required knowledge, skills, and abilities to perform the essential duties and responsibilities of the position.
