Full time FlexStaff is seeking a bilingual Chinese (Cantonese and/or Mandarin) Assistant Director of Patient Services to join our client's Licensed Home Care Services Agency clinical leadership team. Salary: $100,000 - $130,000/year Location: Flushing, Queens Travel: Ability to travel within the service area for RN field supervision. This role will help oversee day-to-day clinical operations, support the supervision and development of nursing staff, and ensure that the agency’s clinical, quality, and regulatory expectations are consistently carried out across interdisciplinary teams.
Responsibilities Assist the Director of Patient Services in overseeing daily clinical operations, implementing clinical policies and workflows, coordinating supervisory coverage and case assignments, and resolving operational barriers. Serve as a clinical escalation resource and assume responsibilities during the Director's absence. Maintain continuous readiness for New York State Department of Health surveys and Managed Care Organization audits.
Coordinate clinical record preparation, tracer reviews, staff education, and follow-up activities; assist with responses to findings; and implement and monitor Plans of Correction under the direction of the Director. Provide day-to-day leadership and support to RN Supervisors, Visiting RNs, and other assigned clinical staff. Participate in recruitment, interviewing, onboarding, orientation, competency assessment, workload distribution, coaching, and performance evaluations.
Monitor completion of required annual performance assessments and in-home observations, as applicable, and recommend corrective or disciplinary action to the Director and Human Resources. Oversee routine chart audits and monitor the timeliness, accuracy, and completeness of assessments, plans of care, medical orders, supervisory documentation, progress notes, incidents, complaints, and other required records. Analyze findings and clinical indicators to identify trends, provide staff education, improve workflows, and support Quality Assurance and Continuous Quality Improvement initiatives.
Monitor the clinical appropriateness, quality, safety, and continuity of patient care. Review new admissions, reassessments, changes in condition, service changes, hospital transitions, and discharge planning. Support the resolution of high-risk, urgent, complex, or hard-to-serve cases and conduct or coordinate field visits when additional assessment or intervention is needed.
Collaborate with physicians, hospitals, insurance companies, MCOs, care coordinators, operations teams, patients, and families to resolve routine authorization, documentation, service-delivery, and continuity-of-care concerns. Monitor clinical caseloads, staff productivity, assessment and visit timeliness, open orders, overdue documentation, staffing coverage, and quality indicators. Prepare reports, facilitate clinical meetings and case conferences, follow through on action plans, and support emergency preparedness, infection prevention, complaint and incident management, and other clinical projects assigned by the Director Qualifications & Skills Minimum of three (3) years of nursing experience and one (1) year of home health experience is required.
Clinical management or supervisory experience preferred. Licensed and registered in NYS as a Registered Professional Nurse Graduate of an accredited school of nursing with a diploma, baccalaureate, or master's degree Demonstrated thorough understanding and knowledge of the Federal, State, and local laws/regulatory guidelines governing the operation of a home health agency is required. Excellent clinical, home visiting, and judgement skills Strong organizational skills and high attention to detail.
Excellent written and verbal communication abilities. Proficiency with computer software and a willingness to learn EMR/EHR systems. Language: Bilingual proficiency in English and Chinese (Cantonese and/or Mandarin) is required.
Travel: Ability to travel within the service area for RN field supervision.
