Coordinates the care of a caseload of patients with medium to high psychosocial acuity by collaborating with treating clinician to review and ensure quality of patient treatment plans; advising, guiding, and/or coordinating services to improve care coordination based on quality principles; using advanced knowledge to recommend patient referrals for moderately complex cases; developing and maintaining case management policies and procedures to ensure optimal and appropriate member utilization and engagement of services; and solving problems concerning patient treatment plans and follow-up appointment documentation.
Enables compliant services by maintaining up-to-date awareness of organizational, state, and federal guidelines as well as regulatory policies, standards, and procedures, to ensure compliance; and proactively submitting written and electronic documentation, records, and reports (e.g., charting) according to department, federal, and state regulations. Qualifications
Minimum one (1) year of experience in a leadership role with or without direct reports. Masters degree in Social Work or a related field AND minimum three (3) years of experience in social work, or a directly related field. Licensed Clinical Social Worker (Georgia) required at hire
