To provide documentation for discharge planning, psychosocial, treatment plan, UR review, and care coordination for patients. Intake phone calls and assessment patient for appropriate programs. Required QualificationsBachelor's Degree In Social Work.

AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) -REQUIREDPreferred QualificationsHospital experience. Case management experience.
