Working as part of a collaborative behavioral health team, you'll play a vital role in promoting healing, recovery, and hope through individualized treatment and coordinated care.If you're passionate about making a meaningful difference in the lives of others while working in a mission-driven, faith-based organization, we'd love to welcome you to our team.The Behavioral Health Social Worker/Therapist provides comprehensive behavioral health services to patients experiencing mental health and behavioral health concerns. This role conducts psychosocial and risk assessments, develops individualized treatment plans, provides individual, family, and group therapy, and collaborates with physicians and the interdisciplinary care team to support positive patient outcomes. The Behavioral Health Social Worker/Therapist also coordinates discharge planning, facilitates referrals, maintains accurate clinical documentation, and ensures continuity of care while delivering compassionate, patient-centered services.Key ResponsibilitiesConduct comprehensive behavioral health assessments, including psychosocial evaluations and risk assessments.Develop individualized treatment plans with measurable therapeutic goals based on patient needs and clinical findings.Provide individual, family, and group therapy utilizing evidence-based therapeutic interventions.Evaluate patient progress and modify treatment plans based on ongoing clinical assessments.Document assessments, treatment interventions, patient progress, and outcomes accurately while maintaining patient confidentiality.Collaborate with physicians, nurses, case managers, and interdisciplinary treatment team members to develop and coordinate patient care plans.Participate in multidisciplinary treatment team meetings by providing clinical recommendations and patient progress updates.Develop comprehensive discharge plans in collaboration with patients, families, and the interdisciplinary team, ensuring appropriate outpatient appointments and community resources are arranged prior to discharge.Coordinate referrals to appropriate internal services and community behavioral health resources to promote continuity of care.Identify and appropriately intervene in cases involving suspected abuse or neglect, demonstrating sensitivity to age-specific and patient-specific concerns.Demonstrate knowledge of the involuntary commitment process and assist with appropriate interventions when indicated.Maintain therapeutic relationships with patients and families while promoting recovery, resilience, and emotional well-being.Support departmental quality initiatives and participate in process improvement activities.Perform other duties as assigned.Knowledge, Skills & AbilitiesKnowledge of behavioral health assessment, treatment planning, and therapeutic interventionsAbility to conduct comprehensive psychosocial and risk assessmentsAbility to develop individualized treatment plans and coordinate appropriate referralsKnowledge of individual, family, and group therapy techniquesAbility to collaborate effectively within an interdisciplinary treatment teamKnowledge of behavioral health documentation standards and patient confidentiality requirementsStrong clinical judgment and critical thinking skillsAbility to recognize and appropriately respond to crisis situations, including suspected abuse or neglectKnowledge of discharge planning and community behavioral health resourcesKnowledge of the involuntary commitment processExcellent verbal and written communication skillsAbility to establish therapeutic relationships with patients and families from diverse backgroundsStrong organizational, time management, and documentation skillsBasic computer proficiency, including Microsoft Office and electronic medical record systemsAbility to read, write, and communicate effectively in conversational EnglishCommitment to continuous learning, professional development, and process improvementEducationMaster's Degree — RequiredField of Study: Social Work, Psychology, Counseling, or a related behavioral health fieldExperienceTwo years of relevant behavioral health experience — RequiredOne year of experience in a healthcare setting — PreferredLicensure & CertificationsLicensed Bachelor Social Worker (LBSW)Basic Life Support (BLS) — PreferredWhy Join AdventHealth?At AdventHealth, you'll find more than a career—you'll discover a calling.
