Full-time | Monday-Friday 8am-4:30pm How you’ll make an impact in this role Assess and Evaluate Patients: Conduct comprehensive behavioral health assessments for hospital patients to identify mental health conditions, substance use disorders, and immediate care needs. Develop Therapy Plans: Formulate personalized, evidence-based treatment and therapy plans in collaboration with the multidisciplinary medical team. Provide Crisis Intervention: Deliver immediate crisis intervention and stabilization for patients experiencing acute distress, including rigorous risk assessments for suicidal ideation.
Deliver Health Coaching: Offer tailored, one-on-one behavioral health coaching to support patients in developing coping strategies and achieving long-term wellness goals. What minimum requirements you’ll need Licensure / Certification / Registration: BLS Provider preferred. American Heart Association or American Red Cross accepted.
Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Counselor preferred. Social Worker preferred.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
