Certifications
✓Current American Heart Association (AHA) Basic Life Support (BLS) certification.
✓In addition to the essential functions of the job listed below, employees must maintain timely completion of all required education, trainings, and certifications as assigned and in accordance with Bozeman Health policies, department-specific requirements, applicable certification program
Education
✓Bachelor’s Degree in Community Health, Medical Social Work, or related field
Master’s of Social Work (non-licensed) preferred
Qualifications
✓Six (6) months of Community Resource experience
✓Demonstrate sound judgment, patience, and maintain a professional demeanor at all times
✓Exercise tact, discretion, sensitivity, and maintains confidentiality
✓Learn current and new computer applications and office equipment utilized at Bozeman Health
✓Strong interpersonal, verbal, and written communication skills
✓Analyze, organize, and prioritize work while meeting multiple deadlines
✓Demonstrate the Bozeman Health SPIRIT values of: Service, People, Integrity, Respect, Improvement, and Teamwork.
One year community resource management experience preferred

About the role

The Psychiatric Services Patient Navigator assists with patients who have complex psychosocial needs and require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. Participates in the development of a discharge plan of care for high-risk patient populations.
