Licensure
Indiana Pharmacist license, Board Certified Oncology Pharmacist (BCOP), preferred.
Education
✓Graduate of an accredited college of pharmacy, Bachelor or PharmD
✓Doctorate degree.

You have successfully completed PGY1 Pharmacy Residency and PGY2 Pharmacy Residency in Oncology, preferred
