About the role

This is an exciting opportunity to provide specialized care to patients with complex heart failure while collaborating with heart failure physicians, APPs, nurses, and multidisciplinary teams. The ideal candidate is passionate about cardiology, enjoys managing complex patients, and values autonomy in a highly collaborative academic environment.

What you’ll do
Evaluate and manage patientsEvaluate and manage patients referred to the Heart Failure Bridge Clinic
Develop individualized treatment plansDevelop individualized treatment plans and collaborate with physicians on complex cases
Assess volume status andAssess volume status and determine appropriate interventions, including IV diuretic therapy when indicated
Coordinate transitions of careCoordinate transitions of care between inpatient and outpatient settings
Provide clinical support andProvide clinical support and guidance to HFBC nursing and support staff
