The Care Coordinator provides case management services in a manner consistent with the philosophy and objectives of Cedar springs hospital for his/her assigned case load. He/She serves as the liaison between the facility Treatment Team and the patient's support person(s), collaterals, etc. The Care Coordinator is the primary responsible party for discharge planning in the outpatient department and is responsible to ensure the patient's discharge is a safe and seamless transition to a less restrictive level of care.

Staff must demonstrate flexibility for reassignment to units providing care to varied populations including children, adolescents, adults, and dually diagnosed patients in an outpatient setting.

Qualifications Education Bachelors degree in Human services or related field required. Masters Degree preferred.

  • Three years prior experience working with a psychiatric mental health population required. Experience in an outpatient psychiatric setting preferred. Licensure/certification: None Competencies: Ability to consistently meet and manage patient expectations. "Doing the right thing, at the right time, for the right person and have the best outcome" (Service Excellence) Ability to provide timely and appropriate care specific to patient needs (Provision of Care) Ability to provide care appropriate to patient's developmental level and needs (Age Specific).
