Care Coordinator, RN

The Care Coordinator is a Registered Nurse who is assigned to a specific clinical care area, facilitating the plan for inpatient/outpatient care by using a collaborative interdisciplinary approach. The Care Coordinator will focus on anticipation, integration and coordination of clinical care and discharge planning services necessary to meet the needs of patients and to optimize clinical and resource outcomes. The Care Coordinator collaborates with the interdisciplinary team in assessing patient needs and developing the plan of care.

The Care Coordinator will also act as an advocate for the patient, physician and hospital relative to compliance with federal, state and third party utilization management regulatory requirements. What You’ll DoServes as a central communication hub between hospitalists, EDs, bed placement/transfer center, nursing, and ancillary servicesAssists with daily census management and throughput optimizationCoordinates ED admissions and patient placement, including acceptance and service assignmentSupports real-time patient flow from ED to inpatient unitsCoordinates care for hospitalized patients to ensure progression from admission through dischargeCommunicates patient status and care priorities across interdisciplinary teams (MDs, APPs, Nursing)Promotes adherence to clinical pathways, workflows, and regulatory standardsIdentifies system issues impacting patient flow and supports operational improvement effortsThis role is not a Case Manager position.

The focus is on operational coordination and patient flow optimization.
