Care Manager RN at Providence Holy Cross Medical Center in Mission Hills, CA. This position is Full-time and will work 8-hour, Variable shifts. The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient centric care management services for at risk patients in an acute care environment.
Accountabilities include assessment and planning, coordination of care, resource utilization management and/ or review, discharge planning, documentation of interventions, regulatory compliance and patient advocacy. Associate's Degree in Nursing degree/diploma upon hire upon hire: California Registered Nurse License California Fire and Life Safety Card - National Organization within 30 days of hire 2 years Acute care experience in a Medical Surgical or Inpatient setting. 1 year
Experience in care management or utilization review in any setting or, Upon hire successful completion of TIPS program or Case Management Orientation Program
Bachelor’s Degree in Nursing or higher within 3 years of hire Case management certification (ACM or CCM)
