Care Partner - LPN

The Care Partner-LPN responsibilities include: Works in collaboration with and under indirect coordination and oversight of the RN Care Partner Coordinators to assist with contacting patients and providers telephonically to improve the health, quality of health care services and experience of the populations served by the network. Reviews daily Excel reports, reaching out to patients and providers and supporting the efforts of BayCare Physician Partners.

Performs other duties as assigned. Potential travel within the BayCare service area using own transportation. Position details: Location: Hybrid Status: PRN, As Needed.

Days will vary. Day shift. Weekend Requirement: Occasional On Call: No Certifications and Licensures: Required LPN (Licensed Practical Nurse) License – State of Florida Required Driver’s License – State of Florida

Required Technical Diploma – Licensed Practical Nurse

Required 1 year as LPN Required 3 years in Healthcare Preferred 2 years in Family Practice Preferred 1 year with Health Insurance Preferred 2 years in a Medical Office
