Job Duties Oversee population to ensure timely progression of care, safe discharge planning, and appropriate post-acute services using evidence-based criteria. Identifies patient clinical conditions, testing and value based clinical interventions that can be completed in the ambulatory space with appropriate follow-up. Apply clinical criteria to monitor medical necessity, reduce avoidable days, and resolve barriers to care in collaboration with physicians, nursing, and utilization management.
Collaborates in the development and implementation of performance improvement activities in collaboration with care management, physician, utilization management. Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. Monitor length of stay, readmissions, and throughput metrics; identify trends; and contribute to initiatives that improve efficiency and patient outcomes.
Bachelor’s Degree Nursing 5 years experience in-patient acute care hospital Excellent communication (oral and written) required to facilitate reviews and discussions with providers. RN - Licensed Registered Nurse_NJ - State of New Jersey Upon Hire
Master’s Degree Nursing Knowledge of care and utilization management Strong interpersonal and organizational skills to manage case load and communicate with multi-disciplinary team members. Ability to work independently and set priorities to ensure appropriate use of hospital resources. Ability to lead individuals and groups.
Physical Demands Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking.
Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
