The Case Manager will provide case management services to clients and families participating in the Center for Hope program.

What you’ll do

Receive client and familyReceive client and family referrals and conduct initial needs assessments
Partners with clients inPartners with clients in case planning and goal setting, continuously evaluating and recording progress made towards goals
Assist in vetting variousAssist in vetting various community programs and agencies to ensure a trauma-informed and culturally appropriate environment and service delivery
Conduct other forms ofConduct other forms of case management, including: helping clients to complete various service and program applications, conducting home and community visits, engaging clients with empathy and compassion, etc
