Responsibilities Case Manager - Part-time - (Bachelors in Social Work) - Behavioral Health – Dubuque, IA Horizon Health is seeking a Case Manager (Part-time) for our behavioral health program at UnityPoint Health-Finley Hospital in Dubuque, IA. The Case Manager is responsible for coordinating services, developing care plans, and advocating for clients to ensure they receive appropriate resources and support. This role works collaboratively with clients, families, healthcare providers, and community agencies to assess needs, monitor progress, and promote positive outcomes.

A Bachelor of Social Work (BSW) is required, along with strong communication, organization, and problem-solving skills.

• Conduct client assessments to identify needs, risks, strengths, and goals Develop, implement, and update individualized service or care plans Coordinate referrals to community resources, behavioral health services, housing, financial assistance, and other supports Participates as an active member of the interdisciplinary and treatment teams. Attend Unit and Hospital meetings as directed Monitor client progress and follow up to ensure services are effective and timely Advocate for clients and help remove barriers to care or services Maintain accurate, timely documentation in accordance with agency and regulatory standards.
