Case Manager BSW

What We OfferAs a Case Manager BSW at Novant Health Huntersville Medical Center you will participate in the complex discharge planning process. Huntersville Medical Center is an award winning 151 bed hospital in Huntersville, a family-friendly town in the Lake Norman area. This part-time position offers you more flexibility in your schedule while still offering part-time benefits.

The schedule will rotate with 3 8 hour shifts and one full weekend every 6 weeks. You must be able to orient on a Monday - Friday basis for 4-8 weeks depending on experience. Novant Health offers you leadership support to help grow your career in the healthcare field.

Participate in complex discharge planning process as a valued member of the healthcare teamCollaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plansCollaborate with RN Case Managers for guidance with medically complex casesServe as a resource for discharge planning and care coordination for psychosocially complex cases, working with MSW Case Managers as neededResponsible for facilitating coordination of careMaintain current knowledge of available community resources and post acute care optionsAccepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of Novant Health What We're Looking ForEducation: Bachelor's Degree in Social Work from school accredited by the Council on Social Work Education is required.

Two or more years professional social work experience or two or more years healthcare experience, preferred. Case Management or related experience, preferred. Additional Skills (required): Weekend and holiday coverage.

Must work one full weekend every 6 weeks. Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding. Why Choose Novant Health?
