$20.86 - $26.19 per hour Pay Transparency: The above reflects the anticipated hourly wage range for this position if hired to work in New Jersey. The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. Core Responsibilities Eligibility & Intake: Screen and verify client eligibility for the HRSA Ryan White HIV/AIDS Program grant funding and renewals.
Assessments: Complete initial and ongoing psychosocial and medical assessments using standard acuity scales. Care Planning: Develop, monitor, and update comprehensive Individualized Service Plans (ISPs) with multidisciplinary teams. Adherence & Counseling: Provide treatment adherence counseling, health education, and crisis intervention.
Referrals & Advocacy: Coordinate access to medical providers, housing, food, and transportation support. Documentation: Maintain accurate records in electronic health record systems and databases like CAREWare
Bachelor¿s or Master¿s degree in Social Work, Nursing, Public Health, or a human services field.
Typically 2+ years of professional case management experience, ideally working with HIV/AIDS, chronic illness, or behavioral health.
