Education
✓This position requires a Bachelor's Degree in Social Work or a Social Work-related field.
Qualifications
✓You are passionate about helping individuals access healthcare and social services that improve their health and quality of life
✓You have an interest in supporting individuals living with HIV and helping reduce barriers to treatment and ongoing care
✓You understand the importance of risk reduction, treatment adherence, and patient education in promoting positive health outcomes
✓You have strong knowledge of or interest in HIV services, entitlement programs, and community resources
✓You are comfortable providing supportive counseling and connecting patients with appropriate healthcare and community-based services
✓You have strong case management, organization, and follow-up skills
✓You are able to maintain accurate and timely documentation while managing multiple patient needs and referrals
✓You communicate effectively and work well as a member of an interdisciplinary healthcare team
✓You understand the importance of HIPAA, confidentiality, and protecting sensitive patient information
✓You are comfortable communicating with patients, families, healthcare professionals, government agencies, and community organizations
✓You are dependable and responsive when addressing verbal and written requests from patients and members of the healthcare team
✓You value collaboration and enjoy working with professionals from different disciplines to coordinate comprehensive patient care
✓You approach patients with compassion, respect, patience, and an understanding of their individual circumstances
✓You are committed to helping patients become more self-sufficient and successfully navigate healthcare and community resources
✓You are organized and able to follow departmental scheduling, documentation, and case management requirements
✓Candidates must demonstrate knowledge of case management standards and the ability to function effectively as a member of an interdisciplinary team.
✓Strong communication, organizational, documentation, counseling, and follow-up skills are important for success in this role.
✓The Medical Case Manager must also demonstrate a strong commitment to HIPAA requirements, patient confidentiality, professional communication, and timely coordination of patient services.
Experience and knowledge of HIV, entitlement programs, and community resources is preferred.
A strong preference is held for Bi-Lingual speakers of Spanish. preferred
