What We Offer As a Case Manager MSW at Novant Health Matthews Medical Center you will serve as a leader of the patient assessment and complex discharge planning process. You'll work with a wide variety of patient populations, supporting multiple units. Matthews Medical Center is an award winning 177 bed hospital in Matthews, a growing suburb of Charlotte ranking #7 on "Best Places to Move in NC" This part-time weekender position offers you the perk of working 24 hours each week and earning 8 hours of incentive pay. A weekender position offers you work/life balance while still giving you part-time benefits. You must be able to orient full time Monday - Friday for a minimum of 4 weeks prior to moving to weekend only training.

Serves as a leader of the complex discharge planning process and a valued member of the healthcare team Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plans Collaborate with RN Case Managers for guidance with medically complex cases Serve as a resource for discharge planning and care coordination for psychosocially complex cases Responsible for facilitating coordination of care Maintain current knowledge of available community resources and post acute care options Accepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of Novant Health What We're Looking For

Master's Degree in Social Work from school accredited by the Council on Social Work Education, required.
