Days | Full - time How you’ll make an impact in this role Orchestrate the overall plan of care from admission to discharge, integrating patient and family preferences into a cohesive strategy across the entire continuum of care. Serve as the clinical expert in assessing and treating complex needs, including legal issues, neglect, substance abuse, crisis intervention, and psychosocial adjustment. Pro-actively identify and dismantle barriers to care, advocating for the internal and community resources necessary to ensure patient safety and health equity.
Maintain a continuous dialogue with the care transition team to reevaluate health plans and provide expert guidance on medical record documentation, complex level-of-care recommendations, and post-discharge options. What minimum requirements you’ll need Licensure / Certification / Registration: Clinical Counselor Preferred. Social Worker specializing in Clinical Practice credentialed from the Florida Board of Clinical Social Work, Marriage and Family Therapy and Mental Health Counseling preferred.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
