Role Overview As a Case Manager, you'll be a vital member of our patient-centered care team, ensuring seamless coordination and advocacy for patients and their families. Your role is to identify, assess, and plan care strategies that promote safety, quality, and cost-effective outcomes.

Qualifications What You Will Need Required Qualifications Bachelor¿s degree in nursing or Master¿s degree in Social Work. Minimum 3 years of clinical experience. Valid, unrestricted State of Michigan license as a Registered Nurse (RN) or Licensed Social Worker (LMSW).
Excellent verbal communication, written documentation, and interpersonal skills. Strong problem-solving, analytical, and decision-making abilities. Proficiency in computer skills and knowledge of healthcare systems.
Certification in Case Management (CCM) or Accredited Case Manager (ACM). Experience in discharge planning, home health care, rehabilitative medicine, or managed care.
