Licensure
✓Current Hawaii State license as an Occupational Therapy Assistant.
Certifications
✓Current certification as a Certified Occupational Therapy Assistant (COTA) from the National Board for Certification in Occupational Therapy, Inc.
✓Current certification in Basic Life Support at the Healthcare Professional Level.
Qualifications
✓standing, walking, finger dexterity, seeing, hearing, speaking, squatting, lifting usual weight of 15 pounds up to 50 pounds, pushing/pulling usual weight of up to 50 pounds, carrying usual weight of 15 pounds.
✓Frequent: sitting, stooping/bending, lifting usual weight of 15 pounds, pushing/pulling usual weight of up to 50 pounds.
✓Occasional: kneeling, crawling, twisting, climbing stairs, walking on uneven ground, static gripping of an object for prolonged periods, frequent gripping of an object, reaching above/at/below shoulder, repetitive arm/hand motions.
One (1) year related clinical experience in area of assignment preferred.
