Education
✓High School Equivalent / GED (Required)
Bachelor's Degree in Social Work, Public Health or in related field (Preferred)
Qualifications
✓1+ years as a Community Health Worker in a clinical and or community setting (Required)
✓1+ years progressive work experience in a healthcare environment (Required)
✓Experience working multiple healthcare data sources that include Electronic Medical Record application(s), registration and scheduling systems, and medical claims systems (Required)
✓Experience facilitating telephonic patient outreach (Required)
✓Competent in all practice areas. (Required proficiency)
✓Basic medical terminology. (Required proficiency)
✓High integrity level and ability to maintain confidentiality of sensitive data. (Required proficiency)
✓Excellent verbal and written communication skills (Required proficiency)
✓Proven time management and organizational skills. (Required proficiency)
✓Demonstrated excellence in customer service with all stakeholders (Patient, employer staff, providers, etc.). (Required proficiency)
