and Responsibilities As our Clinical Ambulatory Pharmacist, you will serve as an integral member of the ambulatory specialty care team, partnering with physicians, advanced practice providers, and clinic staff to optimize medication therapy for patients with complex chronic diseases. You will also help support the development and growth of specialty pharmacy services while delivering safe, evidence-based, patient-centered care.
Every day you will work closely with patients, specialty pharmacies, and interdisciplinary healthcare professionals to facilitate timely therapy initiation, improve medication adherence, monitor treatment response, and identify and resolve medication-related problems. You will coordinate specialty medication management activities including medication access, prior authorization support, laboratory and safety monitoring, immunization assessment, patient education, and ongoing clinical follow-up in collaboration with the care team.
You will be expected to: Partner with physicians, advanced practice providers, and clinic staff to optimize medication therapy. Assess immunization needs and provide patient education. Conduct ongoing clinical follow-up with patients in collaboration with the care team. Work with patients, specialty pharmacies, and interdisciplinary healthcare professionals to support timely therapy initiation.
Help improve medication adherence and identify and resolve medication-related problems. Serve as a clinical resource for providers and staff. Contribute to quality improvement initiatives.
Help develop workflows and implement best practices for this new and evolving specialty pharmacy program. Adapt processes to meet the needs of patients, providers, and the organization. Demonstrate flexibility, initiative, and collaboration while helping shape the future of ambulatory specialty pharmacy services.
Support the continued growth and success of the specialty pharmacy program.
