The Clinical Care Coordinator works under the direction of the CSP Team Leader and in cooperation with Mental Health Clinicians and/or the CSP Team to provide assessments, interventions, and support to Emotionally Disturbed or Chronically Mentally Ill adults and/or children. The Clinical Care Coordinator provides a safe and supportive environment for clients, completes functional assessments, plans for interventions, plans and organizes interdisciplinary team meetings, links clients with services, monitors client progress towards meeting identified individual goals, and functions as an advocate for client rights.
The Clinical Care Coordinator provides clients with the training of critical skills identified as necessary for the individual to function in his/her living, working, learning, and social environments. The Clinical Care Coordinator shares on-call services with the CSP Case Management staff on a rotating basis. Providence caregivers are not simply valued – they’re invaluable.

Bachelor's Degree Prefer degree in behavioral health or education field (public, social services, health education, etc).
Experience working in a behavioral health.
