The LTACH Clinical Liaison is responsible for transitioning patients from the acute care environment to Mosaic LTACH, St. Joseph. The Clinical Liaison will advocate for referred patients in the pre-admission process by collaborating with discharge planners to assess patient needs.
The CL will serve as a resource for healthcare professionals through fostering relationships within medical facilities. With clinical expertise and problem-solving skills, the CL will coordinate care plans with physicians and families. Additionally, the CL will assist in the facilitation of patient transfers and address barriers to admission.
The CL will develop and maintain new and current relationships with various areas of Mosaic Life Care Hospital, Clinics and PAC providers. This position reports to the LTACH President. Responsibilities Assesses potential patient's condition and past medical history and obtains appropriate documentation to support the appropriate post-acute care eligibility.
Knowledgeable of payor coverage requirements pertaining to LTACH criteria. Provides an overview of the appropriate post-acute care service. Communicates with hospital staff to establish a coordinated plan of care that reflects the appropriate post-acute care service to meet the patient's needs.
Collaborates with the attending physician and care management team to effectively manage the hospitalized patient and ensure that transfers to LTACH are expedited as soon as possible. Maintains a strong understanding of LTACH criteria to serve as a consultant to other professionals and referral sources by providing clinical expertise. Consults and collaborates with physicians as appropriate.
Assists in planning and executing educational offerings for physicians, case management, hospital staff and community regarding LTACH services. Participates in committees as assigned Acts as liaison between hospital and physicians to resolve problems, provide information, and maintain positive relationships. Follows all applicable federal and state regulations, including HIPAA, compliance and employment regulations.
Complies with all Care Management and Mosaic Life Care policies and procedures. Education Graduate from school of nursing. - Preferred Work Experience 3 Years - Clinical experience in post-acute care - Required 3 Years - Relevant Healthcare Marketing/Liaison Experience - Required Knowledge of Post-Acute Care Landscape - Required Licenses and Certifications Registered Nurse (RN) - State Licensure/Or Compact State Licensure - State Licensure/Or Compact State Licensure in state, depending upon designated work location - Preferred within 90 Days Basic Life Support (BLS) - Preferred within 90 Days Advanced Cardiac Life Support (ACLS) - IF working in an inpatient area in which ACLS is required - Required within 90 Days Travel Requirements intermittent to area PAC providers and clinics - Required Qualifications Skills and Abilities Essential Technical/Motor Skills Hand/eye coordination; good hearing; computer skills/telephone skills, ability to read, write, and speak clearly.
