Licensure
✓Active and unrestricted pharmacist license in the state of Georgia, or eligible and able to obtain licensure upon hire.
Education
✓Bachelor’s Degree graduate of an ACPE-accredited school of pharmacy or higher in Pharmacy Required
✓Graduate of an accredited School of Pharmacy.
Qualifications
✓Completion of a pharmacy practice residency or
✓2 years of clinical experience required
Completion of a PGY-1 Pharmacy Practice Residency and PGY-2 Solid Organ Transplant Pharmacy Residency or at least three (3) years of equivalent experience in Solid Organ Transplant preferred.

Participate in multidisciplinary inpatientParticipate in multidisciplinary inpatient rounds and contribute to clinical decision-making with the transplant team
Monitor and adjust immunosuppressionMonitor and adjust immunosuppression, antimicrobial prophylaxis, and other transplant-related medications in addition to chronic disease state management
Conduct thorough medication reconciliationConduct thorough medication reconciliation at transitions of care and assess for drug interactions, adverse effects, and adherence issues
Develop, implement, and monitorDevelop, implement, and monitor individualized pharmacotherapy plans for both inpatients and outpatients in accordance with institutional protocols and best practices
