✓Driver's abstract or evidence of good driving record may be required based on area of assignment.
✓Post-graduate clinical residency accredited by the American Society of Health-System Pharmacists.
✓Current certification in CPR at BLS/HCP level.
Current certification as a Certified Diabetes Educator (CDE) preferred.
Education
✓Doctor of Pharmacy degree.
Qualifications
✓Standing, sitting, stooping, bending, walking, finger dexterity, seeing, hearing, speaking.
✓Frequent: pushing/pulling usual weight of up to 40 pounds of force, lifting or carrying up to 30 pounds, reaching above, at and below shoulder level, frequent gripping of an object.
✓Continuous: repetitive arm/hand motions.
✓Occasional: kneeling, crawling, climbing stairs, walking on uneven ground, squatting, static gripping of an object for prolonged periods, pushing/pulling up to maximum 50 pounds.
✓Operate pagers, computers, printers, typewriters, telephones, fax machines, and other office equipment.
✓Valid U.S. driver's license required in accordance with Hawaii state law rules and regulations.
✓Experience to demonstrate ability to communicate professional expertise to all levels of the health care team.
Clinical ambulatory care pharmacist experience preferred.
