Job Details The Clinical Pharmacist oversees the medication distribution system for their area of responsibility. In addition, the Clinical Pharmacist identifies, prevents, and resolves drug therapy problems by ensuring each medication is indicated, effective, safe, and convenient for the patient.

Doctorate Doctor of Pharmacy or RPH (see below under experience) Knowledge of pharmacokinetic principles required for medication dosing Monitor patients for response to medications Identify and document sentinel/serious events, near misses, medication errors and adversedrug reactions in the electronic reporting system Promote and assure compliance with the hospital formulary and policies and proceduresdefined by the P & T Committee.
