and Responsibilities As our Tele-Pharmacist supporting the California market, you’ll help our doctors and nurses safely prescribe and dispense medications to help our patients feel better and recover from their illness or injury. Every day you will meticulously review every prescription in your queue for safety, critically assessing for potential drug interactions, correct dosing, patient allergies, or any irregularities.
You'll then authorize prescriptions for the pharmacy technician, performing a final check before dispensing medication to the nursing station, ensuring expert oversight in every process. To excel in this role, you must enjoy learning new concepts and researching information in real-time to provide solutions for real people. You should also continually look for ways to go above and beyond for your patients, delivering an exceptional experience along with exceptional care.
Reviews, verifies and interprets all physician orders received using the patient profile. Screens all medication orders for optimal medication usage and intervenes when appropriate within the standards of the clinical programs and client hospital protocols. This includes monitoring for medication allergies, drug interactions, renal and hepatic function, age-appropriate drug selection and dosage, duplicated orders, injectable compatibilities, concentrations, and administration rates.
Uses pharmacokinetic skills to determine dosing and to monitor patients’ responses to selected drugs under order of the prescriber or by approved medical staff protocols in order to assure safe and consistent use of designated drugs. Participates in continued education in pharmacy practice in order to meet regulatory requirements and organizational goals. Supports and assists in meeting client hospital goals and specifications for order entry/verification, medication preparation and clinical programs.
Reviews, understands, and responds as appropriate in a timely manner to communication from System Director, Manager(s), and/or Supervisor(s) of Telepharmacy and from client hospital liaisons. Gives thorough, concise communication reports at time of order review or otherwise when appropriate detailing incomplete assignments and potential patient concerns to monitor in order to ensure continuity of medication therapy.
