Facilitates the development and ongoing management of systems that promote desirable patient outcomes through the use of safe, cost-effective medication therapy.

Minimum requirement of Bachelor of Science (BS) or Doctor of Pharmacy (PharmD) in pharmacy degree from accredited school of pharmacy.
Requires a current pharmacist license from state of practice
Minimum 3 years of experience in area of practice

ASHP Accredited PGY-1 Pharmacy Practice Residency preferred. Preferred
Preferred
Two or more years clinical experience and a PGY-1 residency is preferred. Preferred
