About the role

Autonomously functions as a member of the multidisciplinary healthcare team to provide case management, therapeutic crisis intervention, biopsychosocial assessment, therapy and counseling, referral, treatment planning, and other clinical social work services within their practice area. Maintains expert knowledge of available community resources to address the diverse needs of patients, groups, and families.
