✓Health and Wellness Coach Certificate within 12 months of hire
✓Advanced knowledge of health behavior change theories and models, including motivational interviewing (showing minimum of 16 hours MI or other health behavior training, CHES certified or equivalent certification).
Current certification in wellcoaches or equivalent accredited program. preferred
Education
✓Masters degree in health education, education, social work, behavioral sciences, health sciences, or nursing.
Qualifications
✓Minimum two (2) years of (full time equivalent) experience with health behavior change/health coaching.
✓Minimum two (2) years of (full time equivalent) experience in an HMO setting or other health care setting.
✓Knowledge of adult learning theories and models.
✓Demonstrated customer-focused service skills.
✓Demonstrated ability to multi-task effectively.
✓Ability to conduct real-time resource and information searches using Internet.
✓Microsoft Office competency.
Minimum two (2) years of experience with addicted populations (preferably tobacco-cessation). preferred
Minimum three (3) years of experience with health behavior change/health coaching. preferred
Minimum three (3) years of experience working in an HMO setting or other health care setting. preferred

Knowledge of best practices for tobacco-cessation. preferred

About the role

Deliver telephone-based interventions promoting wellness, self care and chronic condition management using behavior-change approaches. As the recognized behavior change expert, act as a referral and resource for providers, support staff and others throughout the region. Offer members appropriately tailored resources and referrals within the KP system and outside community.
