In addition, specific functions may change from time to time:Demonstrates standards of performance (ownership, teamwork, communication, compassion) that supports principles of service excellenceCommunicates with internal and external stakeholders using problem-solving skills to facilitate patient placement and transfers into and out of Cape Fear Valley Health SystemReceives physician orders on new admissions/transfersMonitors bed availability and coordinates bed placement, transfers, and dischargesMaintains a list of patients awaiting transfer into Cape Fear Valley Health SystemServes as a liaison for patients, families, physicians, and staff related to patient transfers, bed availability and patient flow issuesFacilitates effective utilization of technology available for expediting patient flow through the health systemCollaborates with Nursing Leadership, Care Coordination, and individual Units to facilitate patient throughputOther duties as assignedMinimum QualificationsThe following qualifications, or equivalents, are the minimum requirements necessary to perform the essential functions of this job:Education and Formal Training: High school diploma or equivalent requiredEMS credential preferredWork
Computer experience requiredHospital and/or business office experience with specific focus in patient logistics preferredEPIC experience preferredKnowledge, Skills, and Abilities Required: Ability to communicate needs and concerns in a constructive and positive manner.
