The Community Health Educator supports community health strategy by coordinating, implementing, monitoring and evaluating community health programs, screenings and initiatives including the Better Health program. A typical day as a Community Health Educator may look like: Works collaboratively with hospital leaders, clinical departments, community organizations, public health agencies, physicians, community partners, and other stakeholders to identify and address health disparities and social drivers of health and improve access to health and wellness resources Facilitate development of goals objectives, strategies, action plans, and performance measures Serve as primary hospital liaison to community health coalitions and collaborative initiatives to work on and align the CHNA, CHIP and SIP development, implementation, tracking and measurements Develop partnerships that advance priorities identified in the CHNA Work as an active participant in the Community Health and Better Health programming to ensure that programming impacts metrics designed to support departmental, CHIP, and SIP goals Works with executive lead, to develop and manage the hospital’s comprehensive CHNA process and timeline Develop educational programs in alignment with our Medical Advisory Panel on the most current information available, and facilitate programs/classes on those topics with either yourself or an approved speaker leading the program/class Maintains records related to education, screenings, Better Health Member initiatives, and provides data reports monthly or as requested Work collaboratively with staff to help define departmental goals while developing outcome measures with timeframes and targets To be considered for this opportunity, you must have: Bachelors degree in public health, education, health administration, health education, or related field Previous experience working with community stakeholders and community members of diverse backgrounds This position requires some evenings and weekends Preferred: Bi-lingual (Spanish/English) Master's in public health, education, health administration, health education, or related field Certified Health Education Specialist (CHES) Three or more years of experience working in community health education or a related public health area Scheduling Requirements FT Day Shift This position is based out of the RWJ Fitness & Wellness Center, NJ, deploying to community locations in Mercer and other surrounding counties as the programming schedule dictates
