✓High School Diploma, GED or equivalent
Associates Degree or higher in Social Sciences, Healthcare or related field of study, 2 plus years of experience in healthcare setting preferred.
Qualifications
✓Knowledge of healthcare information technology.
✓Knowledge and understanding of basic medical terminology Knowledge of customer service principles.
✓Knowledge of discharge planning, utilization management, and case management.
✓Knowledge of Microsoft Office (Microsoft Outlook, Excel and PowerPoint) Understanding and knowledge of social determinates of health and health disparities.
✓Skill in communicating effectively with all levels of employees.
✓Skill in understanding basic principles of verbal and non-verbal communication.
✓Skill in mathematical computations.
✓Ability to work productively with other agencies in the community.
✓Ability to be alert and able to concentrate on many different items.
✓Ability to work with people of all social, economic, and cultural backgrounds.
✓Ability to establish and maintain a professional relationship with physicians, supervisors, and other hospital staff.
✓Ability to work calmly and compassionately with patients, families, staff, providers and visitors who may be emotionally distraught, angry or upset.
✓Ability to deal with highly stressful, difficult and emotional situations.
✓AND Experience as a Community Health Worker
✓OR Experience as a Medical Assistant
✓OR 2 plus years experience in a patient care or public health setting.
✓Able to work effectively both as a member of a team and with a high degree of independence.
