You’ll need
Education
✓High school diploma or equivalent required.
Qualifications
✓Must be able to speak, read, and write in English.
✓Strong understanding of chronic health conditions, behavioral health, and strategies that support self-management.
✓Excellent customer service, organizational, and follow-up skills.
✓Skilled in motivational interviewing and patient engagement techniques.
✓Ability to communicate confidently and effectively with patients, families, physicians, hospital staff, and other members of the health care team.
✓Demonstrates professionalism, creativity, discretion, diplomacy, and persistence when working with diverse populations and stakeholders.
✓Strong verbal and written communication skills, with the ability to document clearly and communicate effectively by phone, in person, and through electronic systems.
✓Proven ability to work collaboratively in a team-based, interdisciplinary environment.
✓Able to present information clearly to patients, families, hospital staff, health care providers, and community partners.
✓Strong interpersonal skills with the ability to build relationships, support collaboration, and contribute to problem-solving across teams.
✓Flexibility to work varying hours as needed to meet department requirements.
✓Comfortable navigating information systems and moving throughout the hospital environment as needed.
✓Proficient in Microsoft Office applications, including Word, Excel, and Outlook.
Prior experience in home health, community case management, public health, or a related setting preferred.
Experience working with community resources, entitlement programs, and third-party reimbursement preferred.
Bilingual skills highly desirable. preferred
